Job description
The Early Intervention Service Coordinator (SC) is a professional case management position responsible for assuring the provision of Early Intervention services to all eligible children from birth to three years of age and their families in accordance with the individualized Family Service Plan. The SC is responsible for providing a full range of case management services designed to meet the child’s developmental needs, by ensuring access to, and coordinating and monitoring resources and services. This work involves the application of problem-solving techniques, providing support to maximize service delivery and achieve service plan goals, monitoring consumer behavior and the delivery of services and interacting with agencies which comprise of the service network for the individuals being served.
