ERG Home Health Provider, Inc is actively seeking a full-time, experienced, Registered Nurse to fill the Case manger position in our Ventura, CA home health office.
Starting Hourly Rate: Based on experience
Schedule: Monday - Thursday 9am - 5:30pm & Friday 9am - 4pm
Submitting candidates must be able to commute daily to our Ventura office.
Qualifications:
- Current state RN license
- Current State Driver's License with proof of auto insurance
- Current CPR certification
- Excellent skills in communication, management, organization, and problem-solving
Responsibilities:
- Maintaining compliance with all policies and procedures
- Abiding by Medicare and state regulations
- Providing daily direct patient care, home visits and clinical assessments
- Providing supervision to field staff such as LVNs, LPNs, and Home Health Aides
- Experience using electronic medical records software
- Make patient SOC visits as needed
- Patient staffing
- All other RN case management duties
Job Type: Full-time
Pay: $35.00 - $65.00 per hour

- Home health: 1 year (Required)
- Hospice: 1 year (Preferred)
License/Certification:
- RN (Required)
Willingness to travel:
- 25% (Preferred)
