Under general supervision this position uses Case Management Guiding Principles in partnering with patients and physicians to assure access, services, and transition for the continuum of care. Works as team member with health care professionals, community resources, liaisons, and families to provide coordinated admission and discharge planning. Advocates and supports the case management process using the Physician-Centric Care Management Model as a guideline. Partner in identifying and achieving timely patient care needs with implementation of the plan of care that is appropriate, resourceful, and valuable. Effectively utilizes tools that measure, monitor, and manage quality and stewardship of patient care resources.

Hours: Full Time, 1.0, Days, 40 Hours Per Week

Required qualifications:

  • Nursing degree/diploma
  • Upon hire: Alaska Registered Nurse License
  • 2 years Clinical nursing experience.

Preferred qualifications:

  • Bachelor's Degree in Nursing -OR- All Registered Nurses without a minimum of a Bachelor’s degree in Nursing must obtain a Bachelor’s degree or higher within 3 years of hire. Any previously agreed upon effective date for obtaining a Bachelor’s degree in nursing between individual and facility will remain in effect. Any collective bargaining contract with specific time frames for obtaining a Bachelor’s degree in nursing will supersede the above point and will remain in effect. Exceptions will be considered on a case-by-case basis at each facility by the most senior administrative nursing leader in consultation with the Human Resources leader. RNs employed prior to 11/1/17 are encouraged to obtain a Bachelor’s degree or higher degree in Nursing, but obtaining the degree will not be a condition of employment.
  • 3 years Clinical nursing experience.
  • 1 year Utilization Management-review and Discharge-planning experience
