Job description
Education and Formal Training Required:
- Completion of a registered nurse associate or diploma nursing program from an accredited school of nursing. Bachelor’s Degree preferred or if a Social Worker must be at least MSW with LMSW or LCSW preferred
Experience Required:
Five years of clinical experience.
Prefer experience with Microsoft word and Outlook.
Experience Preferred:
Case Management Certificate
PAS certificate or comparable for ECF placements
Training in MCG reviews /criteria
Bereavement and/or Medical Ethics Counselor
Licensure Required:
If R.N. must be currently licensed in the State of Indiana as a Registered Nurse.
Certificates current for all other requirements.
Other:
- Excellent verbal and written skills necessary in order to regularly and effectively interact with hospital staff, physicians, patients, families and third party payers regarding case management issues.
- Ability to concentrate and pay close attention to detail for up to eighty percent of work time when collecting and analyzing data.
- Strong knowledge of multi-disciplinary care planning, as well as nursing practice and procedure; basic knowledge of the utilization and quality review process.
- Ability to organize and prioritize.
- Ability to rapidly learn computer skills.
- Ability to take call for emergency problems.
- Must be able to walk and stand up to 80% of work time.
Working Conditions:
- Works partially in a normal office environment with no exposure to dust, excessive noise or extreme temperatures.
- Exposure to contagious diseases.
- Exposure to blood and body fluids: Exposure Control Level I.
