The Community Based Case Manager-FPP provides services for the Family Preservation, Reunification, and Diversion Programs. Case Manager will be responsible for all casework and therapeutic interventions.

Essential Job Functions
Begins services upon the receipt of the referral contingent upon availability.
Upon receipt of referral, initiates contact with the referred family within twenty-four (24) hours for IFPS referrals or seventy-two (72) hours for FACTS, FRS and Diversion referrals.
Meets with the family to assess the capability for program services to maintain safety of the child and family, as well as the family’s willingness to participate with program services within twenty-four (24) hours for IFPS referrals or seventy-two (72) hours for FACTS, FRS and Diversion referrals.
Contacts the Cabinet within the seventy-two (72) -hour assessment period to notify of the safety of the child, the family’s acceptance of services, and to arrange the ongoing communication and contact between Uspiritus and the referring DCBS worker throughout the duration of services to the family.
Consults with the DCBS referring worker, at least weekly, throughout the duration of services and more often if necessary.
Education and/or Experience
Bachelor's degree (B. A.) from four-year College or university in Social Work, Psychology, or a closely related field and at least one year, full-time employment working with multi-problem families and familiarity with crisis intervention, developing behaviorally specific goals and the ability to teach skills.
