PATH is seeking candidates passionate about helping others make a positive change in their lives to join our ICMS Team as the Case Manager at our Los Angeles office.

ABOUT OUR ICMS TEAM
PATH’s Intensive Case Management Services (ICMS) Program provides services to individuals experiencing homelessness with chronic medical conditions and co-occurring disorders and who are high utilizers or the hospital and outpatient system.

WHAT IS A CASE MANAGER
Case Managers are part of a team of community service providers and PATH staff that work to ensure tenants are moving towards achieving their individual goals. Within the Kaiser Permanente Project Home program, Case Managers are responsible for providing field-based, direct-client services designed to link clients to permanent housing, improve health outcomes and decrease emergency health system use. Responsibilities include:

  • Meeting with tenants regularly to provide strengths-based case management and service coordination, such as coaching around independent living skills, employment assistance and linkage to community providers for services
  • Collaborating with tenants to create person-centered plans that address short-term and long-term goals and the follow-up necessary to reach these goals
  • Consulting with the team to conduct crisis and risk assessments and provide crisis intervention services focused on enhancing the tenants’ ability to independently problem solve, utilize effective coping skills, and manage and self-coordinate own care
  • Mediating and advocating on behalf of the of the tenant to assist with obtaining and keeping stabilizing resources such as housing, healthcare benefits, and transportation

WHAT YOU BRING
We’re looking for candidates that:

  • Demonstrated knowledge of and/or experience with advanced case management techniques including critical time intervention, trauma informed care, harm reduction strategies, crisis intervention techniques and motivational interviewing
  • Ability to maintain and execute confidential information according to HIPAA standards
  • A high level of tolerance and understanding for individuals who present for services with urgent and multiple case management and health needs
  • Ability to work effectively both independently and as part of a team
  • Strong written and verbal communication skills
  • High level of motivation and self-starting demeanor with the ability to coordinate multiple projects and tasks simultaneously in a fast-paced, high-pressure environment

PREFERRED QUALIFICATIONS

  • Bachelor’s Degree in a relevant field
  • Permanent supportive housing experience
  • HMIS experience

MINIMUM QUALIFICATIONS

  • One (1) year of experience working in homeless services, mental health, substance use or co-occurring disorders, or with vulnerable populations providing intensive case management services

MANDATORY REQUIREMENTS
Regardless of qualifications, candidates must:

  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Provide proof of full COVID-19 vaccination
  • Successfully complete the following as a condition of hire:
  • Tuberculosis Test
  • Background Screening
  • Drug Test

DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:

  • Have reliable transportation
  • A valid driver’s license
  • Proof of insurance and ownership for personal vehicles used during work duties
  • The ability to qualify for PATH's insurance coverage

Pay: $21.64 - $26.44 per hour
