Job Title: Bilingual Case Manager
Job Description: Brown Family Health Center is looking for a compassionate individual to serve as advocate for persons living with HIV
About Brown Family Health : Brown Family Health Center is an exemplary rural health center. Founded in 1990, our clinic provides primary health care services to the residents of Deep East Texas, inclusive of HIV/AIDS.
A Day in the Life as a Case Manager: Provides a range of client-centered services that link client with health care, psychosocial and other services to ensure timely, coordinated access to appropriate health and supportive services, continuity of care, and on-going acuity of the client’s needs. Medical Case Management provides initial comprehensive assessment and acuity scale (determines level of client need and appropriate contact) and personal support systems; development of a comprehensive, individualized service plan; coordination of the services required to implement the plan; and periodic evaluation and revision of the plan as necessary over the life of the client.
Qualifications to be a Case Manager: No special skills are required but some requirements mean you have the potential to be a great teller:
- Bilingual Spanish
- Requires valid Texas Driver’s License, appropriate driving record and auto liability insurance that meets the minimum required by the State of Texas Statues.
- A College Degree and one (1) year social services experience
- Requires computer skills.
- Mental and physical health sufficient to meet the demands of the position.
