Case Coordinator

We are Aspire Living and Learning, a non-profit human services agency making a meaningful difference in the lives of adults and children with autism and other disabilities.

Aspire Living & Learning is currently seeking a Case Coordinator is to oversee and ensure the quality and comprehensiveness of record keeping for the individuals we serve, ensuring all regulatory requirements are met and that our services are of the highest quality. The Case Coordinator will be responsible for supporting the overall success of individuals by maintaining regular contact (via phone, email, mailings, and meetings) with guardians, parents, residential providers and clinical staff, and understanding the profile and history of each individual served to ensure that appropriate goals are developed and implemented. The Case Coordinator will attend and participate in all relevant coordination meetings both externally and internally, communicating the overall goals of the department and individuals supported. The Case Coordinator will process Incident Reports when appropriate, review behavior data as needed, participate in Aspire Living and Learning's Case Review Committee (CRC), and complete all mandatory trainings to ensure skill maintenance and development.

Our ideal candidate:

• At least 4 years of relevant professional experience.
• Bachelor's Degree in Psychology, Counseling, or a related field strongly preferred.
• High School Diploma required.
• Excellent verbal and written communication skills.
• Empathy, enthusiasm, and a passion and dedication for helping others.
• Basic computer skills, experience using Therap a plus!
• Personal vehicle and proof of 100,000/300,000 liability auto insurance.
• Valid Driver's License with at least 3 years of driving experience.
• Successful completion of a CORI, BEAS, and reference check.
