Position Overview: The Care Coordinator develops meaningful and person-centered relationships with the individuals served through our Family Living Program and their family members. Additionally, the development of professional relationships and collaborative partnerships with foster providers within Envision’s child and adult shared living/foster care network, as well as with outside professionals on the individual’s service team is also imperative for the support of children and adults receiving services.

DUTIES AND RESPONSIBILITIES:


Care Coordinators carry a caseload of individuals, of which the position oversees coordination of care services and supports to ensure the foster families have what they need to support the child or adult in their care. As part of the Care Coordinator’s role this position supervises foster families, oversees compliance with regulatory standards for DD certified foster parents, and ensures that coordination for services to the individual are followed through on by the foster families. Care Coordinators assist in training the foster parents on the individual specific needs of the individual upon initial placement in the foster family home, and as needed on-going. This position enables Care Coordinators to work collaboratively with individuals, foster families and service teams to establish strategies when needed to best support the individual in developing appropriate, positive skills while providing the foster families with support on how to best meet the individual’s needs to promote healthy, safe, and stable placements.


MINIMUM EDUCATION/EXPERIENCE REQUIREMENTS

  • Associates Degree (minimum qualification)
  • Bachelor’s degree (standard qualification) in social work, psychology, or a related field.
  • At least 3 years in the field working with individuals with developmental disabilities, mental health diagnosis or co-occurring developmental disabilities and mental health diagnoses
  • Experience providing support to individuals in acute mental health settings, hospitals, or residential treatment centers
  • An insured, reliable vehicle for possible on-the-job use
  • Valid driver’s license with no more than 5 points
  • At least 2 year’s supervisory experience
  • Proficiency in computer programs including Word, Excel, Outlook

PREFERRED EDUCATION/EXPERIENCE


  • LSW or MSW, LISW, LPCC
  • Experience in the foster care system, trauma informed care practices, and family systems.
  • Experience as a mental health technician
  • Experience with applied behavioral analysis
  • Experience with functional behavior assessments
  • Experience in supervising Foster Parents
The ideal candidate will have an interest and experience in working with individuals from child through adulthood with development disabilities, mental health needs and trauma histories. The candidate must be able to multi-task, supervise others, work effectively in teams and work in a sometimes fast-paced environment.
