Description and Responsibilities:
The Central Community Health Partnership (Central CHP) is a collaboration of five premier human service organizations in Central Massachusetts that has been selected to provide Behavioral Health Community Partner services through the MassHealth program. For people with Behavioral Health Challenges finding the right combination of services and knowing how to access them can seem to be an overwhelming task. Central CHP stands ready to help MassHealth members with complex needs navigate the confusing array of choices. The four organizations – AdCare, LUK, Inc., Open Sky and Venture Community Services – bring a wealth of experience helping people in Central Massachusetts reach their health and life goals.


The Behavioral Health Care Coordinator will provide outreach, engagement, assessment, and person-centered treatment planning for individuals engaged and enrolled in the CCHP Behavioral Health Community Partners Program. The Care Coordinator serves as a point of contact for integrated care coordination, and care transitions in collaboration with individuals served, medical and behavioral health providers, pharmacies, various social service agencies and other agencies and supports as indicated.

Schedule for this position is a hybrid model, which includes remote work along with office and community visits. The hours are predominately Monday-Friday first shift. and are designed to meet the needs of people served.


Other Key Responsibilities:

  • Participate in the development and implementation of program goals, objectives, and work plan tasks that are directed toward accomplishing the Mission and Vision of the CCHP.
  • Utilize person-centered framework to identify the enrollee’s and/or caregiver’s goals, preferences, desired level of involvement and address any cultural considerations or interpreter services needed.
  • Interface with enrollee’s referral agency, behavioral health, medical providers, and other collaterals to gather clinical information (including all existing medical, behavioral health and treatment plans) to create a comprehensive and person-centered Integrated Treatment Plan (ITP)
Qualifications:
  • High School Diploma/GED or equivalent
  • Reliable transportation to visit participants in the community as needed is required.
  • Experience in community-based behavioral health support programs as a community health worker, peer specialist, or recovery coach.
  • Experience working with adults with mental illness/substance use disorders, experience with accessing local resources, navigating health, behavioral health, and/or substance abuse treatment systems.
  • Excellent communication, documentation, time management, and organizational skills strongly preferred.
