POSITION SUMMARY:

The Behavioral Care Manager (BCM) is responsible for coordinating medical and psychiatric services for patients who are shared by Kedren Primary Care and Kedren Behavioral Health to ensure a holistic & whole-person approach is taken towards providing a health home for all eligible patients. The BCM works as part of healthcare team including clinic manager, physicians, nurse practitioners, physician assistants, case managers, referral coordinators, medical assistants, social workers, front desk clerks, and enrollment and outreach specialist to provide comprehensive care in a non-judgmental manner. Supports the ongoing patient-centered care services for Patient Centered Medical Home (PCMH).

REQUIRED SKILLS AND QUALIFICATIONS:

1. Bachelor's degree (B. A.) from four-year college or university in Human Services.

2. Experience with the psychiatrically disabled population preferred.

3. Interest in working in a primary care clinic with medical staff strongly encouraged.

4. Familiarity with Community Health Clinics (FQHC) and PCMH model

5. Minimum 1-2 years of experience as case manager, non-profit setting highly desirable

6. Knowledgeable of an electronic health record (EHR) system

7. Valid California Driver’s License and proof of personal liability auto insurance as required by California State Law

ESSENTIAL JOB FUNCTIONS:

1. Provide care management services to patients after FQHC practitioner (primary care physician, NP, PA) obtains consent and refers patient to care manager to begin services

2. Provide health home care coordination patients at KCCC who are seen at Behavioral Health by the presence of at least two chronic health conditions, such as "asthma, diabetes, heart disease, obesity, mental condition, and substance abuse disorder; one chronic condition and be at risk for another; or one serious and persistent mental health condition."

3. Assist new KCCC patients who are current Behavioral Health patients in completing KCCC paperwork and releases of information needed to transfer medical care to KCCC

4. Be the system navigator and point of contact for shared Kedren Health patients and families, with patients and families having direct access for asking questions and raising concerns

5. Assist patients during their KCCC medical appointment in understanding the relevant details of the visit, as requested by either the patient or the medical team

6. Schedule appointments for shared Primary Care and Behavioral Health with external medical specialists, as requested by the medical team and explain purpose of appointments in clear and simple language

7. Participate as a member of a multi-disciplinary team including both medical and psychiatric members towards the development and implementation of health home services.

8. Coordinate with KCCC team and community services as needed to ensure that both current diseases, as well as preventative and health promotion needs are addressed as part of the holistic, whole-person, culturally sensitive health home approach.

9. Participate in daily huddle with the care team.

10. Participates in Patient Centered Medical Home Care Team Collaboration (PCMH).

Job Type: Full-time

Pay: $23.10 - $27.50 per hour
