Alternative School Transition Specialist
General Statement of Job

Under general supervision, the employee serves as a liaison between schools and supports students who transfer between the alternative school and traditional/residential schools. A transition specialist performs a variety of tasks to coordinate successful student transitions utilizing both behavioral and academic interventions. In addition to leading a transition team, this position is also integral to providing an effective Multi-tiered System of Support (MTSS), including Social Emotional Behavior (SEB), focusing on data collection and analysis to inform and model data-driven decision-making for students at the alternative school. Reports to the principal of the alternative school.

SPECIFIC DUTIES AND RESPONSIBILITIES

ESSENTIAL JOB FUNCTIONS

Develops and manages transition plans for students who are returning to their traditional/residential school of assignment; plans and implements check-ins with students.

Oversees the Alternative School Level Transition Team process (i.e. team member training, agenda preparation and meeting reminders, and environmental arrangements).

Collaborates and aligns available resources to develop and implement intervention plans with academic and non-academic (behavioral) interventions for each student.

Assists with the collection and analysis of data needed for the purpose of planning, reporting, securing services and making decisions as related to students who are returning to their traditional/residential school of assignment.

Communicates student progress with both parents/guardians and traditional/residential school staff.

Participates in Child and Family team meetings and EC/504 meetings for students who are returning to their traditional/residential school.

Performs other related work as assigned.

KNOWLEDGE, SKILLS AND ABILITIES

  • Knowledge of curriculum and instruction and strong knowledge of effective skills and practices for student success
  • Ability to develop and maintain trust, positive rapport, and effective working relationships with students, families, and staff
  • Ability to encourage and motivate students and monitor their safety and well-being
  • Exceptional collaboration and communication skills, both oral and in written
  • Knowledge of MTSS framework and the ability to analyze data from multiple sources and problem-solve
  • Strong organizational and critical-thinking skills
  • Knowledge of trauma informed practices
  • Educational knowledge from a wide range of experiences, particularly in classroom settings

MINIMUM TRAINING AND EXPERIENCE

Bachelor’s degree with active license and experience in education and/or social work. Proven record of success with 3-5 years of experience.

SPECIAL REQUIREMENTS
Must hold a valid NC Driver’s License and be able to travel between school locations as needed to support student transitions.

TO PERFORM ESSENTIAL JOB FUNCTIONS

Physical Requirements
: Must be able to use a variety of equipment and classroom tools such as computers, copiers, typewriters, calculators, pencils, scissors, and equipment for children with special needs, etc. Must be able to exert a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Due to amount of time spent standing and/or walking, physical requirements are consistent with those for Light Work.

: Requires the ability to observe, compare, and contrast the readily observable, functional, structural, or composite characteristics of various data, people, or things.

Interpersonal Communication
: Requires the ability to speak and/or signal people to convey or exchange information. Includes receiving instructions, assignments and/or directions from superiors.

Language Ability
: Requires the ability to read and prepare a variety of instructional materials, correspondence, reports, handbooks, forms, lists, etc.

Intelligence
: Requires the ability to apply principles of logical or scientific thinking to define problems, collect data, establish facts, and draw valid conclusions; to interpret an extensive variety of technical instructions in mathematical or diagrammatic form; and to deal with several abstract and concrete variables.

Verbal Aptitude
: Requires the ability to record and deliver information, to explain procedures, to follow oral and written instructions. Must be able to communicate effectively and efficiently in a variety of technical or professional languages including medical, legal and counseling terminology.

Numerical Aptitude
: Requires the ability to utilize mathematical formulas; to add and subtract; multiply and divide; utilize decimals and percentages; and to apply the principles of descriptive statistics, statistical inference and statistical theory.

Motor Coordination & Dexterity
: Requires the ability to coordinate hands and eyes rapidly and accurately. Must have minimal levels of eye/hand/foot coordination and ability to handle a variety of equipment and tools.

Visual and Spatial Aptitude
: Requires visual acuity to complete essential duties, including the ability to conceptualize visual representations, size, shape, and color.

Interpersonal Temperament
: Requires the ability to deal with people beyond giving and receiving instructions. Must be adaptable to performing under stress and when confronted with persons acting under stress.

Physical Communication
: Requires the ability to talk and hear: (Talking: expressing or exchanging ideas by means of spoken words. Hearing: perceiving nature of sounds by ear). Must be able to communicate via telephone.
