Position Purpose:

  • To screen prospective residents in both the acute and community setting.
  • Performs on-site clinical and financial assessments to determine appropriateness of admission.
  • Works with residents/families through the admission process to ensure an effective admission.

Qualifications:

  • Graduate of an accredited program- nurse preferred, but willing to interview any discipline with experience
  • Eligible to participate in federally funded health care programs.
  • Proficiency in basic computer functions including Electronic Health Records
  • Knowledge of current federal, state, and local guidelines in the Long-Term Care
  • Must have knowledge of Medicare and Medicaid and secondary insurance program.

Duties and Job Responsibilities:

  • Completing all preadmission forms for all departments.
  • Maintains contact as Community Liaison.
  • Assist with in-services on the admission process and customer service as needed.
  • Assist in maintaining data and tracking information for census, inquiries, admissions, discharges, and referral sources.
  • Provide information on Advanced Directives, Medicare and Medicaid programs, as needed.
  • Give tours, perform admission screenings and handle initial inquires.
  • Work with the Director of Nursing to ensure that all admissions are screened thoroughly and adequately prior to admission. (Pricing of medications, clinical and personal information).
  • Provide all required financial information to the bookkeeping department.
  • Work with the different long term insurance programs on providing information for reimbursement to family/sponsors.
  • Provide Integracare with information concerning medications.
  • Handle monthly report to OBRA for Level Is.
  • Assist in getting Level I completed prior to admission and with a significant change in resident condition.
  • Maintain data on customer satisfaction.
  • Send thank you notes to new inquires and sympathy notes to the families of residents that expired.
  • Assist with all required reporting to ADPH.
  • Introduces the family/sponsors of new admissions to the facility business office for financial information.
  • Act as liaison between the facility and family members during the pre-admission and admission process. Upon admission, refer family members to appropriate Department heads depending on specific need.
  • Complete Medicare notices as needed.
  • Break down medical records for discharged residents.
  • On closed records flag unsigned orders and place the orders in the MD box.
  • Set up medical records with tabs.
  • Scan documents into the medical record
  • Charting and documentation in the electronic medical record as required by facility policies & procedures.
