General Summary: Responsible for the residents’ assessments and written service plans, designed to meet their needs, including coordination and participation in personal care and life enrichment activities. Responsible for outreach and marketing efforts to maintain a census of 92% or higher in the Memory Care Unit working in conjunction with the Marketing Director. Work in cooperation with the Administrator, Human Resource Director, and RN’s.

Essential Functions/Major Responsibilities include the following:

  • Plan, organize and coordinate with appropriate staff the Residents’ personal care routines.
  • Assists RN with Initial Resident Assessments and required follow up Assessments.
  • Maintain all necessary record keeping on computer and in Resident files.
  • Insure all Service Plan changes are communicated to the appropriate parties, i.e. caregivers, families, Administrator, Nurses, etc.
  • Perform staff training in Alzheimer’s and Dementia education to ensure resident needs are met.
  • Keeps Administrator informed of any issues or concerns that arise from employees, residents or their families.
  • Plan and schedule Service Plan meeting with families and Residents.
  • Organize and host monthly Alzheimer’s Support Group for families.
  • To review Physician Orders for Life Sustaining Treatment (POLST), and set up a visual plan for easy accessibility under the guidance of the RN’s.
  • Keep all private resident or employee information confidential.
  • Assist in mentoring and supervision of care staff as needed to ensure the service plan is followed as you have instructed.
  • Oversee the Memory Care Activity Coordinator as they plan, coordinate and implement the resident’s activities, to meet physical, spiritual, emotional, intellectual and social needs.
  • Work closely with the Department Heads to coordinate space, equipment, food and transportation for planned events and activities.
  • Oversee the Memory Care Activity Coordinator as they maintain a safe activities program, including: driving, events, space, planning, mobility, etc.
  • Ensure orientation and training is done for any family member, employee, or volunteer assisting in any part of an event or activity or with any resident in your care.
  • Assist the Marketing Director to achieve and maintain a high occupancy rate in Memory Care through active management of referrals, leads and inquiries using database information and other tools.
  • Work effectively with staff in the Business Office, Housekeeping, Dietary, Maintenance, Caregiving and Nursing to provide the best customer experience possible.
  • Assist the Marketing Director to represent Jennings McCall at Trade Shows and Masonic & Eastern Star functions. Spend time outside the facility as a resource person, information source and spokesperson for Jennings McCall.
  • Welcome visitors and be available to answer questions from residents or guests and direct them within the facility.
  • Assist in conducting IL and AL tours in the absence of the Marketing Director.
  • Perform all Memory Care specific tours when scheduling allows.

· Contacts are both inside and outside the organization. Internal contacts include employees, management, volunteers and residents. External contacts include potential residents, families, visitors, and healthcare professionals.

Job Scope:

This job has recurring work situations with occasional variations from the norm. Duties involve a high degree of complexity due to the nature of all the life care services this entails. Duties are performed under indirect supervision of the Administrator with established procedures and guidelines. Errors in accuracy, judgment, and level of professionalism effect the smooth and efficient operation of the department, ultimately impacting residents, management and staff trust levels.

Specific Skills:

· Excellent interpersonal skills

· Ability to maintain cooperative working relationships with those contacted in the course of work activities. Communicate effectively in English both verbally and in writing

· Skills in reading and math

· Knowledge of principles and practices of Residents’ needs, including the use of various Residents transferring plans and equipment

· Skill in problem solving, and the ability to remain poised in emergency situations

· Ability to understand and work under a variety of situations in a positive constructive manner

· Ability to use and teach creativity in meeting Residents’ emotional and physical needs

· Ability to be organized, set and meet deadlines

· Ability to do many daily tasks via computer, i.e. emails, spreadsheets, etc.

· High degree of integrity in all areas of conduct related too or reflecting on the Organization.

Education and/or Experience:

· High school diploma or equivalent.

· Oregon Administrator’s Certification (active)

· At least two years of previous administrative experience preferred

· At least one or more years of psychology/counseling experience or education preferred

· Valid Oregon drivers license

Job Conditions:

· This job may require hours that exceed 40 per week and a flexibility of hours (i.e. weekends, nights as requested)

· This job may require day and overnight traveling to seminars, trainings, Resident Assessments, etc.

Physical Demands:

· Ability to regularly walk, stand, bend, stoop or sit for extended periods of time

· Ability to use office equipment requiring a high degree of dexterity, i.e. keyboarding

· Work for sustained periods of time maintaining concentrated attention to detail and accuracy

· Work for sustained periods of time at a computer station

· Ability to communicate with various contacts via walkies, phones, etc.

· Ability to work under pressure
