Columbia County Health System is looking for a Patient Advocate! A new position that will be responsible for communicating with patients about their care, focusing on how their care may be improved, as the organization strives to deliver quality and efficient care to our community. This position will work under supervision of the Chief Operating Office, and communicate and interact with patients, patient families, and staff.

Wages for this position are dependent on experience and licensure.

What You Will Own and Improve in this Position:

• Being the point of contact for patients regarding their experience
• Collecting and compiling data on patient experiences to report to patient experience committee, QI committee, and Clinics
• Interpreting patient feedback to improve workflows and practices
• Resolving patient concerns in a prompt and effective manner

What You Will Bring to the Position

• Ability to communicate and interact with all groups of people without judgment and without escalation
• Strong organizational skills to develop collect patient feedback, interpret data, provide feedback to patient care team, and work toward resolution
• Demonstrated empathy towards others
• Deep understanding of HIPAA restrictions when dealing with patient information

About Your Team

You will work with multiple departments throughout the organization, but interact heavily with the Chief Operating Officer, QI department, and Clinics with Columbia County Health System. This team always has the patient at the forefront of their motivation, looking for ways to improve the experience, while addressing limitations to their care. They are ready to collaborate to improve the health care experience of our community.

To Be Successful

This position requires a 2-year degree or equivalent experience in a relevant field. Having your MA, LPN, or RN license is preferred, but not required. Tell us about your experience! Especially anything to do with:

• De-escalation training
• Customer service
• Patient Safety, Just Culture, and HIPAA Compliance
• Fluency in other languages
• Health care experience
