Health Care Assistant – Long Term Condition Management


We are currently looking for an experienced Health Care Assistant to join our small hard working team. This role will focus on long term conditions management.

This is a fantastic role within the practice and we would like you to possess the following skills.

  • Phlebotomy
  • BP
  • Preferably wound care - although not essential


This role will involve Admin work and the ability to recall/monitor patients utilising our clinical system Vision. This role involves working closely with others so excellent communication skills are a necessity.

We are a 3 GP Partnership, including 1 Salaried GP, 2 Practice Nurses, 1 Practice Mental Health Nurse, 1 Community Link Worker, Pharmacy Support, 1 Clinical PA, 5 Patient Care Advisors and a Practice Manager. We currently have around 4, 300 registered patients and are looking to expand. We work in a spacious, purpose build primary health care centre and have District Nurses and Midwives on site.

We are offering an excellent remuneration package with a starting salary £21,709 (FTE) pro rata. We also offer the option to join the NHS pension scheme.

The hours of work will be 20 hours per week - Monday to Friday. Maximum hours up to 25 per week negotiable.
