Mobile Outreach Case Manager

Catholic Social Services Kilian’s Shelter is a Youth and Young Adult (YYA) shelter program for homeless individuals 18-24 years of age who are experiencing homelessness. This population faces numerous challenges, including family estrangement, unemployment, homelessness, and/or behavioral/mental health conditions. The main focus of the shelter program is to help young adults find appropriate housing and provide supportive services as well as case management.

The Outreach Case Manager will provide the resources and support needed to improve the overall health, social, and economic outcomes of the YYA. The Outreach Case Manager is responsible for promoting the program throughout the surrounding areas, targeting YYAs who are at risk of becoming homeless or are homeless and in need of housing stability.

Conducts community outreach to develop resources to support this population. Part of a multi-disciplinary team that provides case management, housing referral, financial counseling and benefits services.

The Outreach Case Manager is responsible for ensuring the functions of the program are met while providing services that are safe, timely, effective, efficient, equitable and client-centered.

Qualifications include:

• BA/BS in Human Services preferred, experience in human services
• Experience working with youth and young adults or homelessness individuals
• Experience in housing stability and Case Management
• Excellent organizational and communication skills
• Reliable transportation required
• Bi-lingual helpful
