We are currently recruiting for a Wound Care Nurse.

Responsibilities:

  • Complete a weekly Tracking and trending of all wounds in the facility.
  • Weekly or as needed rounds with Wound Care MD. Update residents/POA of changes, gain consent for procedure, document care, and send update to DON, Administrator, IDT, and VP of Clinical Ops.
  • Communicate with IDT team the status and changes to wound as needed
  • Review and order treatment supplies as needed (wound vac, dressings)- maintain inventory of wound vacs
  • Complete wound care treatments, initiate, discontinue replace on wound vacs
  • Pressure ulcer prevention program direct oversight
  • Notification to RP and MDs as needed for wound changes with documentation per standard of care.
  • Ensure all wounds are measured up on admission/when acquired and at least every seven (7) days per regulations and LHSC policies and procedures.
  • Recommended and obtain all necessary DME for wound care treatment. Obtain DON/Administrator approval for new equipment purchase or rental as needed.
  • Daily rounds to all units to talk with Teammates about wounds or new areas of concerns. Examine all residents with current wounds or concerns.
  • Ongoing education to all nurses on wound care and dressing changes.
  • Audits on wound care and dressing changes. Correct any concerns, establish a PIP to correct, and notify DON/Administrator.
  • Conduct all weekly body audits on residents, with assistance from Teammates as needed. Participate in all admission body audit as much as possible.
  • Audit all body audits and address any concerns with education and possible PIP and in-service Teammates as needed. Notify DON/Administrator of any concerns.
  • Ongoing education and in-service to Teammates about wound care expectations and quality care practices.
  • Assist Employee Health Nurse with new employment screenings, health screenings, TB testing, vaccines clinics, SREOs, and other services as needed.
  • Orient new Teammates on wound care, etc.
  • Update and develop policies and procedures in accordance with regulations from DHEC, ASHA and CDC as directed by VP of Clinical Ops and/or Administrator.

Qualifications:

  • Graduate of an accredited school of nursing
  • Licensed to practice as an LPN or RN in the State of South Carolina
  • Must maintain current SC LPN or RN license and CPR certification
  • Previous wound care experience required
  • Wound Care Certification (WOCN) preferred
  • Must possess the ability to make independent decisions and authority when circumstances warrant such action
  • Must be able to deal tactfully, cheerfully and patiently with personnel, residents, family members, visitors, and the general public
  • Must be knowledgeable of nursing and medical practices & procedures, as well as laws, regulations, and guidelines pertaining to long-term care
  • Must possess leadership and supervisory ability and the willingness to work harmoniously with professional and non-professional personnel
