Our Hospice Agency is currently searching for a compassionate and experienced License Vocational Nurse for the Hawthorne area and vicinities.
Responsibilities and Duties:
- LVN visits patients at their place of residence and provides bedside skilled nursing care.
- Does medication reconciliation and reinforce pt/PCG/family teachings on medication and safety use.
- Effectively utilizes all resources to assist in meeting patients' needs.
- Reinforces teachings/guides/maintain a safe home and environment for the patient.
- Contributes to providing a level of hospice care according to the plan of care and doctor's orders.
- Complete, timely EMR documentation every after the visit.
- Participate in IDT meetings, when requested.
Qualifications:
- Licensed as a Vocational Nurse in the State of California.
- Fluent in Spanish and English.
- With valid CPR/BLS certification and complete Covid 19 vaccination.
- Valid driver's license, auto insurance, reliable transportation, and willingness to drive to patient locations.
