CSO is looking for a Triage Case Coordinator for our Emergency Services program in Greenfield.

This position will triage service needs for any individual identified in the Emergency Department (ED) who may need additional behavioral health linkages. They will ensure referrals to appropriate services and work directly with the case management team in the ED to ensure collaboration between medical and behavioral health service needs.

This is a full-time position requiring afternoon, evening, and some weekend availability.

Responsibilities

  • Engage clients in the ED identified by the ED staff.
  • Provide individualized support based on person's unique profile including the individual's support system and potential needs identified by the ED and by the individual themselves.
  • Provide assistance in clarifying needs, gaps in services and supports.
  • Promote integration opportunities across CSO services.
  • Provide follow up with internal and external referral sources to ensure linkage occurred and trouble shoot with CSO managers and programs to reduce barriers to access.
  • Assist in providing linkages to housing, transportation supports, and accessing supports for stabilization needs.
  • Practice a holistic and integrative approach to healthcare that includes physical, medical and emotional wellness. This involved an overview of areas of need and social determinants of health.
  • Utilize a person-first approach.
  • Assist with identifying appropriate insurance and benefit application and accessing services of choice.
  • Assist with individual and family advocacy needs including identifying and obtaining entitlements, which meet consumer's needs. Make appropriate referrals to other agencies as needed.

Qualifications

Bachelor's degree in a behavioral health discipline or related field, or equivalent combination of high school diploma or Associate's degree and applicable experience. Demonstrated experience working with adults with mental health challenges including experience navigating the behavioral health system. Good listening skills; ability to foster self-sufficiency of individuals using problem-solving and practical skills; strong familiarity with support groups, education groups, and strong understanding of services in both the CSO continuum and throughout the applicable service area. Excellent oral and written communication skills, and computer competency. Must be able to work independently and follow through with tasks as well as be an integral part of a multidisciplinary team. Must have driver's license and reliable transportation.
