Title: Care Coordinator (case manager) nurse positions within our Primary Care clinic. Shift: Weekdays (no weekends or holidays). Collaborate with support nursing staff and with providers for patient care

- Orientation: on Mondays only - If your traveler misses the compliance deadline, they will pushed to the following Monday
