SUMMARY
The Transitional Case Manager will work with young people identified as needing support by
Harford County Department of Social Services. Serving clients generally between 20 ½ and 21 ½
years old. The Transitional Case Manager will bridge the period of time when the young people
lose the supports of foster care. The average length of support is expected to be 6 months.


ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING

Other duties may be assigned as necessary.
  • Provide intensive case management and clinical interventions to transitional aged youth.
  • Complete assessments for youth in program, as required by the agency to determine
current functioning and to develop client's initial needs and service plan.
  • Identify and develop recommendations for client. Refer and direct services to clients to
include but not limited to housing referrals, educational needs, financial services, and
employment support.
  • Provide identified support services in accordance with the youth's Individual Service
Plan.
  • Conduct weekly in-home or community visits with youth.
  • Support the youth with discharge and aftercare planning
  • Maintain an average caseload between 8-10 youth.
  • Maintain accurate, timely, and complete case notes, service plans, and other
documentation according to agency standards
  • Participate in ongoing trainings and supervision.
  • Maintain client records in Arrow's electronic client information system utilizing assigned
Arrow technology resources. These resources may include but are not limited to desktop
computers, laptop computers, touchscreen devices, and scanners.
  • Utilize (personal) cell phone mobile data hotspot for in-field access to Arrow's client
information system in order to complete documentation in accordance with job workflow.
  • Review client files monthly to access service plan implementation, progress towards
desired outcomes, and future appropriateness of service plan.
  • May be required to drive ACFM vehicles, be insurable with our auto insurance
provider, and complete Arrow's vehicle training.
  • Perform all duties within contract, state and federal regulations, Arrow policy,
and COA standards.

REQUIRED KNOWLEDGE AND SKILLS
To perform this job successfully, an individual must be able to perform each essential duty
satisfactorily. The requirements listed are representative of the knowledge, skill, and/or ability
required. Ability to communicate effectively both verbally and written with internal and
external stakeholders. Ability to deal with standardized situations with occasional variables.
An individual must be able to apply basic math fundamentals with the ability to interpret and
clearly communicate data and concepts.


QUALIFICATIONS, EDUCATION, AND/OR LICENSURE

Bachelor's degree in Human Services or a related field and at least one (1) year experience
working for foster youth. Master's Degree preferred. Frequently required to operate a motor
vehicle.
