The Trans Health Services Navigator is a professional counselor/educator who provides individual and group counseling to adolescents around issues of birth control methods, pregnancy options, STD testing, HIV testing and referrals. The Trans Health Services Navigator’s primary role is to provide assistance to transgender and gender non-conforming (TGNC) patients in navigating medical and supportive service systems including: case management; referrals to needed outside services and care; entitlements screening and enrollment; coordinating services at The Door; assisting patients in planning towards gender-confirming medical interventions (hormones and/or surgery); supportive counseling; and advocacy for TGNC patients involved in outside medical and supportive service systems. Additionally, the Navigator will develop and maintain relationships with community organizations supporting TGNC communities. This position will work closely with AHC and Door staff to ensure that the full continuum of care needs of our TGNC clients are identified and that appropriate services are accessed, coordinated, and monitored.

Responsibilities:

  • Provides individual and group education/counseling with adolescents around hormone therapy, birth control, pregnancy prevention, HIV/AIDS prevention, and making healthy choices.
  • Provide emotional support, crisis intervention, and referrals to patients.
  • Participates in health care delivery and case management, in collaboration with other health center staff, including the clinician, social worker, lab staff and medical assistants; implement, monitor, and update personalized service plans for patients. Document all services in the medical record.
  • Develop linkages with health and service organizations providing services to TGNC New Yorkers; maintain referral resource of TGNC-sensitive health and social service providers in the Tri-State area.
  • Assist in the development of health education materials focused on the health needs and concerns of TGNC communities.
  • Provide HIV education, and pre- and post-test counseling and referrals to adolescents; provide counseling prior to and following a pregnancy test and provides appropriate pregnancy options referrals.
  • Maintain productivity reports and logs; and assist with preparing grant reports.
  • Represents The Door and the mission at schools, health care agencies, community-based organizations, health fairs and community meetings.
  • Responsible for community-based assignments, conducting classroom presentations and small group sessions, and providing counseling and referrals other services and programs at The Door.
  • In conjunction with a NYSDOE/NYCDOE certified teacher, teaches health education sessions with an emphasis on age-appropriate sexual health topics in schools.
  • Maintains accurate records and logs of education sessions and outreach activities.
  • Other duties as assigned by Supervisor.

Qualifications:

  • Bachelor’s degree in Social Work, Health Education, Public Health, Public Administration, or related field. Relevant work experience and some college credits will be considered in lieu of a bachelor’s degree.
  • A minimum of 3 years of experience in family planning, reproductive health care, adolescent health services or counseling.
  • In-depth knowledge of social and health issues affecting diverse TGNC communities; strong knowledge of transgender services and resources.
  • Knowledgeable of and ability to apply positive youth development principles to working with young people.
  • Proficiency in Spanish strongly preferred.
  • Strong computer competency.
  • Strong organizational and communication skills a must.

Work Schedule: Full time, Monday - Friday, 35 hours per week (includes some evenings and weekends)
