As a Support Services Case Manager at Nashville CARES, you will develop relationships with clients and support them through many stages of life. This position is based full-time in the office at our Nashville location.

What You’ll Do

  • Provide compassionate and respectful care and support services to help clients address the complex challenges associated with living with HIV
  • Meet regularly with clients in the office, in clients’ homes, and in the community
  • Ensure clients are able to access all needed support services provided by Nashville CARES, as well as resources in the community
  • Assist clients with connections to medical, social, community, financial, and other needed services
  • Assist clients in developing realistic action steps to achieve their goals
  • Provide support services and referrals to clients who may be socially isolated
  • Advocate for clients within the agency and in external settings, such as medical providers and other agencies they may be engaged with in the community
  • Complete all documentation in an appropriate manner and in a reasonable timeframe according to agency standards to ensure clients do not encounter barriers to service
  • Practice appropriate boundaries with clients and with work to ensure you are able to provide excellent services to clients while preserving self-care

Minimum Requirements:

  • Bachelor’s degree (preference for Bachelor’s in Social Work or other Human Services)
  • Minimum of two years’ experience in community based social or health care services – a supervised BSW/MSW internship may count for one year
  • Proficiency with a variety of technology, including Microsoft Office software (Word, Excel, Outlook), electronic health records (training provided), and general Windows PC and cell phone technology
  • Valid Driver’s License, vehicle, and automobile liability insurance to facilitate local travel required for client/agency-related work

  • We provide a vast array of services, including case management, medical care, housing assistance, nutrition and transportation services, and assistance with insurance and prescription needs.

Pay Range: $37,000-$39,000 annually ($17.79-$18.75 hourly).
