Full-Time Pharmacist - Syracuse, NY


Minimum Qualifications:

  • Bachelor of Science in Pharmacy (Doctor of Pharmacy or a Master of Science degree in Pharmacy desirable)
  • Current licensure as a registered Pharmacist in the state of New York
  • Minimum of two years recent experience in an acute-care setting or one-year recent experience in home infusion therapy(preferred).
  • Experience and strong knowledgebase in intravenous admixture services and intravenous therapy
  • Capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians, nurses and other healthcare professionals
  • Capable of working effectively with others to provide safe and effective drug therapy.
  • Demonstrate ability to problem solve and supervise technical and other support staff.
  • Good organizational and time management skills
  • Participation and membership in professional societies and organizations desirable
  • Excellent verbal and written communication skills
  • Dependable attendance and job performance

Performance Requirements:

1. Providing pharmacy support by:

  • Making appropriate clinical decisions
  • Making appropriate business decisions
  • Functioning autonomously
  • Documenting patient and/or departmental activity clearly
  • Successfully guiding and facilitating staff
  • Communicating clearly and consistently between departments to promote interdisciplinary approach to home care (ie, patient rounds, case conferences)
  • Authoring and performing requested and required in-services.
  • Effectively monitoring staff and outside agencies as necessary
  • Identifying and resolving departmental, operational problems if Supervising Pharmacist is not available.
  • Maintaining a positive and motivating attitude
  • Maintaining good working relations with prescribing physicians
  • Complying with all state and federal employment laws

2. Provides clinical pharmacy services by:

  • Participating in daily rounds
  • Receiving/obtaining physician infusion orders
  • Evaluating patient/customer for safety and appropriateness of drug delivery
  • Monitoring patient/customer clinical records for order complications/concerns
  • Evaluating TPN orders; monitoring and recommending changes, as needed
  • Contacting patients to set up delivery.
  • Managing all pharmacy patients:
  • Adding medical history
  • Adding medical profile
  • Adding appropriate care plans
  • Adding progress notes
  • Performing technical pharmacy activities competently:
  • Compounding
  • Sterile/aseptic techniques
  • Calculation of TPN orders

3. Follows Cleanroom/USP Standards by:

  • Wearing no cosmetics or jewelry (watches, rings, earrings, etc. piercing jewelry included) upon entry into ante-areas.
  • Bringing no food or drinks into or stored in the ante-areas or buffer areas.
  • Being aware of the line of demarcation separating clean and dirty sides and observes required activities.
  • Donning shoe covers or designated clean-area shoes one at a time, placing the covered or designated shoe on clean side of the line of demarcation, as appropriate.
  • Donning beard cover if necessary
  • Donning head cover ensuring that all hair is covered.
  • Donning face mask to cover bridge of nose down to include chin.
  • Performing hand hygiene procedure by wetting hands and forearms and washing using soap and warm water for at least 30 seconds
  • Drying hands and forearms using lint-free towel or hand dryer
  • Selecting the appropriately sized gown examining for any holes, tears, or other defects
  • Donning gown and ensuring full closure
  • Disinfecting hands again using a waterless alcohol-based surgical hand scrub with persistent activity and allowing hands to dry thoroughly before donning sterile gloves.
  • Donning appropriately sized sterile gloves ensuring that there is a tight fit with no excess glove material at the fingertips.
  • Examining gloves ensuring that there are no defects, holes, or tears.
  • While engaging in sterile compounding activities, routinely disinfecting gloves with sterile 70% IPA prior to working in the direct compounding area (DCA) and after touching items or surfaces that may contaminate gloves.
  • Removing PPE on the clean side of the ante-area
  • Removing gown and discarding it or hanging it on hook if it is to be reused within the same workday.
  • Removing and discarding mask, head cover, and beard cover (if used)
  • Removing shoe covers or shoe one at a time, ensuring that uncovered foot is placed on the dirty side of the line of demarcation and performing hand hygiene again. (Removing and discarding shoe covers every time the compounding area is exited)
  • Performing proper hand hygiene, garbing, and gloving procedures according to SOPs
  • Disinfecting ISO Class 5 device surfaces with an appropriate agent
  • Disinfecting components/vials with an appropriate agent prior to placing into ISO Class 5 work area
  • Introducing only essential materials in a proper arrangement in the ISO Class 5 work area
  • Not interrupting, impeding, or diverting flow of first air to critical sites
  • Ensuring syringes, needles, and tubing remain in their individual packaging and are only opened in ISO Class 5 work area.
  • Performing manipulations only in the appropriate DCA of the ISO Class 5 device
  • Not exposing critical sites to contact contamination or worse than ISO Class 5 air.
  • Disinfecting stoppers, injection ports, and ampule necks by wiping with sterile 70% IPA and allowing sufficient time to dry
  • Affixing needles to syringes without contact contamination.
  • Puncturing vial stoppers and spiking infusion ports without contact contamination
  • Labeling preparation(s) correctly
  • Disinfecting sterile gloves routinely by wiping with sterile 70% Alcohol during prolonged compounding manipulations
  • Cleaning, setting up, and calibrating automated compounding device (e.g., “TPN compounder”) according to manufacturer’s instructions.
  • Disposing of sharps and waste according to institutional policy or recognized guidelines.

4. Improves department/company productivity and efficiency by:

  • Monitoring and expediting resolution of prescriptions
  • Identifying system/operational shortfalls and presenting improvement suggestions
  • Developing and training staff to improve work performance.
  • Identify and implement clinical changes.

5. Ensures department compliance with company policies and procedures.

6. Shows teamwork, initiative and accountability by:

  • Volunteering to assist co-workers.
  • Learning about newly introduced products/services: their purpose, assembly, customer education and safety issues
  • Complying with applicable memos and policy changes as issued
