GENERAL DESCRIPTION OF POSITION

Under the supervision of clinic leadership: RN Clinical Support Supervisor, Assistant Clinical Support Supervisor, or RN Regional Clinical Manager, provides direct patient care in clinic and related work as assigned.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Assists in the registration of patients, completes EHR data entry. Supports front office staff with patient registration, scheduling and answering the phones as needed.
  • Assist clinic healthcare providers in providing direct patient care by rooming patients, obtaining patient history and reason for seeking care with in-clinic and telehealth appointments. Maintains strict confidentiality
  • Gathers necessary vital signs from patients and other visit specific details, completes entry in EHR for provider review and signature for entry into patient's chart.
  • Completes patients in-house testing, or care, such as blood draws, capillary sticks, throat/nasal swabs, basic wound care, EKG, removing sutures/staples, ear lavage, eye exams, and eye exams for well child visits, CDLs, and sports physicals.
  • Administers selected medication and treatments to patients as ordered, including injections and immunizations and instructs patients on proper follow-up procedures under the supervision of the provider. Enters medication refill requests for provider review and approval.
  • Uses clinical knowledge to recognize and respond appropriately to urgent/emergent situations per protocol. Completes necessary documentation (EHR/Incident reporting) as needed.
  • Maintains updated immunizations in EHR, and state immunization platform.
  • Maintains the supply stock in exam rooms and lab areas and communicates usage so supplies may be replenished. Checks stock daily for expiration dates.
  • Cleans examination rooms between patients and at close of business day. Adhering to North Country Healthcare practices according to the Infection Control Policy, and safety plan.
  • Anticipates provider's needs related to patient care while following standing order protocols.
  • Performs daily, weekly, monthly cleaning and maintance on CLIA-waived lab equipement, autoclave and medical instruments. Keeps sharps containers, biohazard bags emptied and prepared for pick-up.
  • Performs laboratory procedures and speciment collection for medical and occupational health patients per orders.
  • Keeps own work space clean and tidy, and, as a team member, assures cleanliness of common use areas.
  • Assists and communicates with other NCHC team members, from all departments, when needed to collaborate to support the North Country mission.
  • Adheres to NCHC Standards of Behavior which includes: participating in meetings as required, regular and predictable attendance, continuous learning in both personal and professional development and consistent review, comprehension and reply of company email and related correspondence.
  • Performs other specialized duties as assigned by the department leadership in which they function.
  • Requires adherence to all policies and procedures, including, but not limited to, standards for safety, attendance, punctuality, and personal appearance. Ability to multi-task efficiently and effectively and cope in a healthy way with job stress.
  • Communicates provider initiated information to patient related to care such as test results, follow up instructions and other related patient care needs.
  • Participates in training of new staff to ensure consistency in protocols, policies and safe, quality patient care.
  • Maintains familiarity with the HIV and HCV registries and assists in ensuring accuracy. This is performed weekly.
  • Works with Regional RN to establish and maintain communicable disease reports. This duty is performed weekly.
  • Participates in interdisciplinary team meetings to include clinical pharmacy, Ryan White case managers, clinicians, and behavioral health. This duty is performed weekly.
  • Coordinates with Outreach to work with grant reporting guidelines. This duty is performed daily.
  • Assists in tracking and monitoring patients being cared for by specialty teams, including preparation reports for interdisciplinary team (IDT) meetings when appropriate. This duty is performed weekly.

EDUCATION AND EXPERIENCE

High school, plus specialized schooling and/or on the job education in a specific skill area; plus 12 to 18 months related experience and/or training.

Diploma of completion of education or training as a medical assistant or a related fie

ADDITIONAL SOFTWARE SKILLS

Capability of learning the Electronic Health Records.
