The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities.


Roles & Responsibilities:
As a member of the interdisciplinary care team in this primary care setting, the social worker triages patient care as needed focusing on same-day, short-term assistance to address issues such as mental health assessment/referrals (including suicidal ideation), substance use assessment/referral, transportation, homecare, insurance, housing, and hunger. In this role, the social worker conducts brief biopsychosocial assessments, identifies mutually agreed upon goals, and assists with developing and facilitating a plan. The social worker is responsible for educating patients and care partners about navigating the health system and the availability of relevant resources, coordinating with other MSHP programs, and linking patients/care partners to community organizations and services. The social worker also assesses for high risk patient eligibility for longitudinal care coordination and makes referrals as needed. At this time, the position is partially remote. Return to fulltime on-site presence is anticipated.

Mount Sinai Health Partners (MSHP) is the management services organization developed to implement the Health System’s population health initiatives: programs of care that enhance the individual patient’s experience and health status, improve the health of patient populations, and reduce utilization and healthcare costs. MSHP provides the infrastructure and machinery to support population health initiatives, particularly preventive care, through care coordination, information technology, workflow optimization, and quality reporting. The Department of Social Work Services has taken a lead role in the development of MSHP by providing transitional and longitudinal care coordination services.

LOCATION (for onsite work): 17 East 102nd Street, Manhattan.

Requirements:
  • NYS LMSW or LCSW.
  • Bilingual (English/Spanish) preferred.
  • Exceptional skills in engaging patients and families and in assessment and coordination of resources within families and hospital and community networks for effective management of patient care.
  • Strong clinical assessment skills.
  • Exceptional organizational, collaborative and psychosocial assessment and intervention skills.
  • Interest in being part of an innovative initiative to improve the quality of care by addressing barriers to primary and specialist care, preventable Emergency Department visits and hospital admissions/readmissions.
  • Hospital experience preferred.
  • Proficiency in MS Office: Word, Excel, Outlook. Ability to learn hospital ambulatory scheduling system, electronic medical record and specific care coordination applications.

The Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $70,837.99 - $85,066.00 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
