POSITION SUMMARYThe Social Worker is responsible for performing a wide range of patient care activities for assigned patients. The Social Worker conducts patient assessments, contributes to the patient treatment plan, and the discharge plan; and provides individual, group, and family counseling as indicated. Reports to the Clinical Manager.

Graduation from an accredited School of Social Work (MSW). Master’s Degree required.LICENSURE

One (1) year of experience in health care with home health preferred.CERTIFICATION
