Schedule: Full-time, 40 hours per week
- This position will serve NICU/Birthplace
Under the direction of the Case Management Director, serves as a coordinator of an interdisciplinary approach to guide patients through the continuum of care with focus on discharge planning, psychosocial needs assessment, care/resource management, and provider/insurance issues. Collaborates with physicians, patients, families, and others to facilitate the achievement of desired outcomes, address ongoing needs, maximize patient/family/and provider satisfaction, optimize resources, facilitate timely discharge, assure appropriate length of stay, reduce unnecessary delays and other adverse variances to the delivery of services.
Minimal Acceptable Job Qualifications
Licensure:
- Current Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW) required
Education: Must have a Master or Bachelor of Social Work degree from an accredited college or University.
Experience: Minimum of one to two years of clinical social work experience in a hospital or health care setting which may include the appropriate supervision or internship.
Other:
Demonstrate knowledge of the hospital service theme.
Demonstrates a working knowledge of insurance industry requirements.
Demonstrates strong interpersonal and communication skills.
Must be able to accept responsibility, make independent decisions and interact with professionals.
Must have the ability to manage multiple tasks/projects simultaneously.
