Job description
The Social Worker provides patient and family assessments in order to design effective treatment programs and follows the patient and family through the continuum of services. The Medical Mgmt Social Worker 1 provides supportive counseling to patients and families, assesses patient discharge needs and coordinates discharge services. Relies on education, experience, professional training and judgment to accomplish responsibilities. A wide degree of creativity and latitude is expected. Works under minimal supervision.

- Interactive communication in planning and implementing of discharge process is conducted. Interacts in a positive, competent and respectful manner with patients, families, caregivers and staff, co-workers, physicians and community agencies as evidenced by no verbal or written complaints.
