Southport Center for Nursing and Rehabilitation is seeking a Social Worker to join its team.
The Social Worker provides assessments, evaluations, treatment planning, short term- psychotherapy and social services to individuals, couples, families and groups as clinically determined. The Social Worker participates in clinical staff development, supervision and performance improvement activities as a member of an interdisciplinary team within a primary care medical setting. Most importantly, the Social Worker is a patient advocate and makes every effort to assure that the best interests of the patients are being met.

License:
- LMSW, LCSW, LMHC, LCAT or LMFT (Preferred)
