Seeking a Social Worker for our practice who provides clinical expertise to ensure all patients receive high quality, efficient care. The Social Worker:
- Provides direct social service and counseling to patients/families
- Assists in locating appropriate resources for post-discharge care
- Participate in multidisciplinary team to coordinate the client’s plan
- Cross-trains with team members to serve as back up resource
- Assesses and identifies cases for intervention
- Prioritizes goals and develops plans based on client needs
- Monitors and evaluates outcome of client’s plan
- Proactively identifies cases
- Collaborates with third party payors internal case management/disease management departments as appropriate for discharge planning needs
Education:
- Bachelors degree or Masters degree required
Licensure/certification/registration:
- Current licensure by Texas State Board of Social Work
- LMSW, LCSW or LPC preferred
- BLS issued by the American Heart Association
Experience:
- One year social work experience preferred
