Do you have a passion for caring for others? Do you have a desire to help others achieve their fullest potential? We are in search of a Full Time Social Worker who will be responsible for providing services to meet the social and/or emotional needs of our residents.

• Development, coordination, and participation in family and resident activities designed to promote social interaction.
• Assessing and completing the assigned sections of the (MDS) Minimum Data Set
• Conducting daily resident rounds
• Plan, schedule and implement room visits for resident unable to leave their room.
• Assess, plan and document residents' discharge needs.
• Counsel residents and families in dealing with feelings about death or dying and other emotional, mental environmental or physical limitations.

Qualifications:

• Bachelor of Social Work preferred.
• Previous leadership experience preferred but not required.
• Previous experience in a long-term care environment is preferred but not required.
