WHAT WILL I BE DOING?

CHI is looking for a full-time Senior Case Manager for our Commack location. You will be responsible for all aspects of social work services within the Congregate Family Program. You will ensure that quality housing services are provided effectively and in accordance with guidelines established by CHI, OTDA, Suffolk County Department of Social Services, and other funding sources as appropriate. You will supervise the overall running of the Case Managers at your site. This is accomplished by performing the following functions:

  • Provide crisis intervention as appropriate.
  • Provide regular supervision for Case Managers.
  • Ensure that the program is providing quality services and are compliant with program requirements.
  • Promote sound social work practices through supervision including case consultations, case file reviews, and employee performance appraisals.
  • Ensure the correct and timely submission of all internal and external reports in the manner required by funding agencies and/or CHI (housing logs, client contact sheets, unit inspection sheets, progress notes, authorization forms, admission and discharge sheets, census reports, etc.).
  • Inform Emergency Housing Manager of deficiencies that require additional staff training.
  • Act as a liaison with agencies responsible for referring eligible customers.
  • Adhere to proper use of company resources (cell phones, office supplies, internet, etc.).
  • Always maintain appropriateness.
  • Participate in the hiring process, initial and annual employee evaluations as well as disciplinary measures.

WHAT DO I NEED?

  • Education: BSW or MSW from an accredited university or related field of study required.
  • Experience: 2-5 years’ experience providing management services.
  • Certificates or Licenses: Valid driver’s license.
  • Insurance: Valid automobile insurance.
  • Communication: Excellent verbal and written communication skills. Proficiency in English required. Bi-lingual (Spanish) is a plus.
  • Computer Skills: Basic competency in MS Windows, MS Office, and on the Internet.
  • Math Skills: Must be excellent.
  • Physical Performance: Ability to tour properties, walk distances, climb stairs, etc.
  • Reasoning Ability: Ability to prioritize, make appropriate decisions and judgment calls.
  • Other Skills: Ability to work independently or as part of a team, be highly organized, assume responsibility, work well under pressure, meet deadlines, multitask, be flexible, and maintain a high level of attention to details. Must be even-tempered, have good negotiation skills, and the ability to meet unexpected deadlines. Effective time management skills.
