**Per Federal Health Care Worker Vaccine Requirements, all employees must be fully vaccinated and received booster against COVID-19 or be approved for a valid religious or medical exemption. POSITION PURPOSE/SUMMARY
: Responsible for the management and delivery of clinical care to patients and families in the HSJ service areas in accordance with Federal and State standards and laws. Directly supervises and schedules the assignments of the RN nursing staff. Under the direction of the Chief Clinical Officer, the Team Leader will handle phone calls from RN Case Managers and assist with care coordination as requested, back up the LVN Supervisor in the receipt of patient phone calls for staff that are out of the office, and triage needs appropriately. The Team Leader is responsible for the coordination and supervision of assigned staff, as well as ensuring the compliance of patient visit schedules, quality assurance activities, and coordination of care in the absence of the assigned staff.

1. Manages the assigned staff work schedules, including approval of PTO Overtime, and adjusted work schedules in order to meet patient care needs. Implements Low Census when appropriate and under the direction of the DHCS. Notify the receptionist of assigned staffing for the day (i.e. which per diem staff are working for the day, who is covering RNCM caseloads in their absence, etc.)

2. Ensures that Case management is being carried out in accordance with Hospice Policies

3. Updates staff on changes to policies and procedures as directed by CCO

4. In collaboration with all Team Leads, arranges for patient services: CADD pumps, and palliative modalities as ordered by the Physician and requested by the RNCM. Report all palliative modalities to the coder.

5. Responsible for notifying the CCO and /or Administrator on call of any unusual occurrences or adverse events, and for facilitating immediate follow-up of such occurrences as appropriate.

6. Serves as Clinical Supervisor On-call on a rotating schedule (approximately once a month for 7 days at a time

7. Ensures compliance with the Hospice program plan by assuring quality service, appropriate utilization of resources, and employee, and patient/family safety.

8. Maintains up-to-date patient records so that problems, plans, actions, and goals are accurately and clearly stated and kept in accordance with Medicare, MediCal, State Home Health
Agency and Joint Commission standards.

9. Using clinical assessment skills completes the initial nursing assessment; identifies the physical, practical, and physiological needs of the patient. Assess patients within regulatory
requirements. Perform admission visits and follow-up assessments in a thorough and complete manner as needed.

10. Works with social services to identify the emotional, psychological, and spiritual needs of the patient.

11. Collaborates with physicians to determine the appropriate frequency of nursing visits and the need for a change of level of care (continuous home care, respite, and attendant care, or inpatient admission, discharge for the Hospice Program.)

12. Provides support to families at the time of patient’s death with coordination of services with physician, mortuary, DME, supplier

13. Assists family in making the transition from caregiving to bereavement, in coordination with IDT members

14. Uses expert judgment and functions as a patient advocate, in collaboration with IDT and primary physician, when seeking a change in an optimal care setting for the patient, incorporating patient safety and family caregiving ability.

15. Works with pharmacy, therapists, DME suppliers, and other health care providers in the absence of the case manager to ensure coordination of patient care.

JOB QUALIFICATIONS:

1. Currently licensed to practice professional nursing in the State of California.
2. BSN degree preferred.
3. At least 3 years of dealing with acute care patients preferred. Six months or more of home care or Hospice experience is preferred.
4. Supervisory experience
5. Ability to set priorities, organize and work independently.
6. Ability to learn and assimilate the Netsmart computer software program.
7. Ability to communicate effectively verbally and in writing.
8. Skill in nursing practices with an understanding and knowledge of Hospice care and commitment to the philosophy and goals of the program.
9. Ability to make independent clinical decisions.
10. Knowledge of medications, indications, dosage ranges, side effects, and potential toxicity.
11. CPR Training


OTHER QUALIFICATIONS:

1. Ability and willingness to drive own vehicle, have a valid California driver’s license, and have automobile insurance that meets State standards. All employees are required to show evidence of insurance.
