Position Summary:
Promotes the value of hospice services offered by Montgomery Hospice by maintaining a professional presence in Suburban Hospital and surrounding hospitals to facilitate the referral of patients to Montgomery Hospice GIP services. Provides information/education related to hospice and palliative care to patients, families, and hospital staff.
Duties and Responsibilities:

• Promotes an increased understanding of services and programs to appropriate health-related audiences through one-on-one contact, attends meetings and responds to inquiries.
• Meets with physicians, hospital discharge planners, nursing home social workers, and liaisons to assure services are meeting needs of referred patients and families.
• Attends discharge planning rounds, educational programs, case conferences and staff meetings at hospitals as appropriate.
• Maintains records of all contacts and requests for information.
• Participates in assessing, planning, implementation and evaluating policies to ensure efficient referral processes.

Minimum Qualifications:

• Graduation from an accredited Nursing program. B.S.N preferred.
• Active and current Maryland RN license
• Knowledge of state, federal and regulatory standards, including Joint Commission standards as it applies to hospice organizations.
• Knowledge of Montgomery County and its medical community a plus.
• Must have effective interpersonal skills, excellent written and communications skills.
• Access to and ability to drive a car for local travel to appointments.
• Computer proficiency required.
• Requires mobility to meet demands of outside appointment schedule, including driving within metropolitan area, accessing various offices and medical facilities.

Pay: $36.00 - $56.00 per hour
