The RN Case Manager identifies continuing care needs through assessment, interdisciplinary care rounds and/or requests. In collaboration with patients, families and community services establishes a coordinated discharge plan based on the patient's transition needs. Coordinates continuing care services to include skilled nursing, durable medical equipment and community services. Interfaces with aftercare providers to ensure smooth transition with the post discharge plan of care. Conducts ongoing assessment to determine the continued need for acute care, and appropriateness/timeliness of treatments/procedures to optimize reimbursement.

From Parking Lot A, (smaller lot fills up quick)

• Access the Urann Lobby from parking Lot A.
• Check in with the desk to the left.
• Take the elevator to the third floor.
• Take a right off of the elevator.
• Follow the hallway to the end where you are forced to go left.
• At the locked double doors, press the button on the intercom on the wall to the right. Tell them you are here for a meeting with Kevin Coughlin and they will buzz you in.

From Main Entrance

• Take the stairs directly in front of you to the bottom. (or an elevator is available to the left)
• At bottom of stairs, take a right following the long hallway to the end, Diagnostic Imaging will be on your left.
• Take a left and follow the hallway into the next lobby (Urann lobby).
• Take the elevator from there to the third floor.
• Take right off of elevator.
• Follow to end where you are forced to go left.
