The Registered Nurse Care Manager (RNCM) is a critical part of the health care team and is responsible and accountable to the patients. The RNCM assists clinicians in the development of the plan of care for patients. In this role, the RNCM is responsible for the care management and coordination of a patient panel and acts as an advocate, educator, and resource person for this population. The RNCM addresses the logistical, financial, emotional, educational, and social barriers to care and compliance using the health-team model of care (which includes the whole patient). Working under the direction of the Clinical Manager, the RNCM plans, assesses, and evaluates the patient’s clinical status ensuring treatment plan and medication administration compliance through preplanning activities, care rounds, and patient engagement. The RNCM may need to cover more than one center and team. Collaboration with other RNCMs, nurses, and staff members is required to fulfill the needs of the team. Daily contact with clinicians, health center staff, patients, caregivers, and outside agencies/providers is required.

HOURS/SCHEDULE: The position is 4 hours every day Mon-Fri.


Education:

Graduate of an accredited School of Nursing required. BSN preferred.

Required Certifications, Licenses, Registrations:

Registered Nurse licensed in the State of Maryland or Compact State.

CPR (healthcare worker) certified within 90 days of employment.

Current MD driver’s license

Required Experience:

Two/ Three years of clinical nursing experience.

Prior case management experience is strongly preferred.
