Job Description and Responsibilities

Under the direction of the Director of Assisted Living and Care Management, the principal purpose of this position is to act in the absence of the SALSA (Supervisor of Assisted Living). The designee has the same educational requirements and must be familiar with the responsibilities of the SALSA as they pertain to daily oversight of Assisted Living Services delivered to the residents of Duncaster. Responsibilities include the ongoing assessment of resident needs and assuring that competent, qualified staff delivers those services as directed. This position will coordinate care delivery utilizing the available resources of Duncaster and determine the need for transfer to a more appropriate level of care when ongoing needs can no longer be met. The following description of job responsibilities and standards are intended to reflect the major responsibilities and duties of the job description but is not intended to describe minor duties or other responsibilities as may be assigned from time to time.

  • Coordinating and managing all nursing and assisted living staff providing direct care assisted living services.
  • Supervision of the delivery of direct care services by licensed nurses and aides assigned to assisted living.
  • Working in conjunction with the Staff Development Coordinator, provision of evaluation of the clinical competency of all assigned nursing staff upon hire and annually.
  • Working in conjunction with the Staff Development Coordinator, assure ongoing education needs and requirements for all clinical staff are met.
  • Participate in the program planning, budget preparation and evaluation of services.
  • Participate in the development of annual goals for assisted living, standards of care and policies and procedures concerning the provision of care services.
  • Oversee medication administration for assisted living residents.
  • Make recommendations to the Director of Assisted Living regarding staffing levels and participate in the recruitment, selection and orientation of staff.
  • Ongoing assessment and evaluation of residents as indicated by resident’s health status but not less than every 120 days.
  • Develops with the resident/family and care team, an individualized plan of care.
  • In conjunction with the Scheduling Coordinator, determines daily assignments for nursing assistants to assure that residents’ needs are met and that they are updated and revised as needed. In the absence of the Scheduling Coordinator, schedules assisted living aides to assure sufficient staff on duty each day.
  • Provide regular reports to the Director of Assisted Living regarding any problems associated with the provision of core services, statistical data regarding number of residents served, hours and types of services delivered and need to transition residents to a different level of care.
  • In conjunction with the Duncaster nurse, orients all new Private Duty Aides to the plan of care.
  • This position may deliver direct nursing services to residents.
  • Ensures the Duncaster nurses collect weekly information of services delivered for billing purposes.
  • Monitor monthly budget and review variances with the Director of Assisted Living.
  • Participate in the ongoing Quality Improvement activities of Assisted Living.
  • In the absence of the Director of Assisted Living, submit weekly reports to Service Coordinator regarding the provision of core services, problems and concerns associated with the MRC or AL services. Summary of weekly reports are made available for the Management Committee quarterly.
  • In the absence of the Director of Assisted Living, submit monthly reports to Service Coordinator regarding statistical data to include residents served and services provided. Summary of monthly reports is made available for the Management Committee quarterly.
  • Communicate with practitioners in the outpatient clinic to assure residents’ needs for services are met.
  • In coordination with the outpatient clinic and when the need arises, perform assessment of potential Assisted Living residents.
  • Utilization and superuser of the Matrix system.
  • On-call responsibilities with 24/7 responsiveness, within 30 minutes, to important messages and emergencies.
  • Weekly Dogwood visits with support to the Dogwood nursing and aide staff.

Job Requirements:

Education and Experience

  • Registered Nurse with a valid Connecticut License
  • Minimum of 4 years full time experience with at least 1 year of Home Care/ Community Health experience or BSN with a valid Connecticut License, minimum 2 year’s full-time experience with at least 1-year Home Health/ Community Health experience.
  • Clinical experience within the past 10 years.
  • Must possess a basic knowledge of computers, ability to use Word, Excel and Outlook.
  • Must have the ability to effectively communicate in both oral and written form.
  • Must have the ability to read, write and speak the English language.
  • Must be able to perform the essential job functions with or without reasonable accommodation.

Hours

Monday-Friday, 7:00am-3:30pm with on-call every 2 weeks.
