Practice Highlights:

  • Hours of Operation: Monday - Friday, typically 8a-5p
  • System-wide Athena EMR
  • Onsite care team includes Physicians, Nurse Practitioners, Clinical Pharmacists, Behavior Health Clinicians, Case Managers, RN Clinic Managers, RNs, Medical Assistants, and Diabetes Educators

Candidate Highlights:

  • 2+ years of experience in a primary care setting preferred.
  • NM Nursing License required
  • Maintains current CPR certification
  • Athena EMR experience highly desired
  • Team collaborator and patient advocate

The preferred candidate will have a working knowledge of Patient Centered Medical Home, FQHC and ACO requirements as regards patient quality outcomes.

Position Details:

REMOTE HYBRID POSITION-THIS POSITION OPERATES ON A ROTATING SCEDULE BETWEEN CLINIC AND REMOTE WORK. TYPICALLY ON A 1 TO 2 WEEK ROTATION

SIGN-ON BONUS AVAILABLE

Reporting to the Clinic Manager, the RN Care Coordinator delivers additional medical support and coordination of care under the supervision of a patient's treating provider for an identified high risk, high acuity patient population.

Serves as a key member of the interdisciplinary care team and in collaboration with other providers and team members, develops interdisciplinary care plans,

Provides clinical care in compliance with SCC policy as well as the provisions of the New Mexico Nurse Practice Act.

Acts both internally and externally to facilitate appointments, scheduling and completion of tests of other procedures, and to remove or minimize other barriers to adherence to care.

Documents appropriately and timely in the EMR.

Collects and analyzes data to support development and improvement of clinical best practice and SCC quality programs.

The preferred candidate will have a working knowledge of Patient Centered Medical Home, FQHC and ACO requirements as regards patient quality outcomes.
