Currently seeking a Registered Nurse to make visits to patients in Salt Lake County! Pay is on a per-visit basis and is open to negotiation with management based on years of experience. Scheduling is flexible and availability to take on-call shifts is preferred.

Pay ranges is $60.00 - $120.00 per visit.

Job Summary:

The Registered Nurse provides skilled nursing care to patients in their home under the general guidance and direction of the Director of Nursing, according to the physician’s orders, and in accordance with agency policies and procedures, and following established standards and practice.

Qualifications and Experience:

Ø Currently licensed in the State of Utah as a Registered Nurse

Ø One year of nursing experience, and experience in home health care/hospice recommended

Accountability:

Reports to the Director of Nursing

Typical Physical Demands:

Requires full range of body motion including handling and lifting of patients. Manual and finger dexterity, and hand-eye coordination. Occasionally lifts and carries items weighing up to 30 pounds or greater. Requires corrected vision and hearing to a normal range. Requires working under stressful conditions or working irregular hours. Requires exposure to communicable diseases. Requires a current driver’s license and the ability to drive and access a patient’s home without an assistive device.

Job Duties:

Ø Make the initial nursing evaluation visit by completing a comprehensive history and nursing assessment.

Ø Re-evaluate patient needs based upon patient status and condition.

Ø Initiate the plan of care and make necessary revisions.

Ø Complete the plan of care re-certifications.

Ø Initiate appropriate and preventative and rehabilitative nursing procedures.

Ø Accept staff assignments based upon specific patient needs, and family capabilities.

Ø Perform general nursing care to patients. Administer prescribed medications and treatments in accordance with nursing standards.

Ø Observe, record, and report patient condition and reaction to drugs and treatments to physicians.

Ø Arrange for patient testing and admissions.

Ø Respond to and refer incoming telephone calls.

Ø Instruct patient and family regarding medication, treatment regiments, and self-care activities.

Ø Maintain and review patient records, charts, and other pertinent information.

Ø Prepare list of medical supplies needed.

Ø Attend required meetings and participate in committees as requested.

Ø Participate in professional development activities and maintain professional affiliations.

Ø Maintain patient confidentiality.

Ø Perform related work as required.

Ø Supervise and consult with LPNs as necessary.

Ø Inform the physician and other personnel of changes in the patient’s condition and needs.

Ø Participate in in-service programs.

Ø Perform on-call duties as assigned.

Ø Follow all Agency policies and procedures as outlined in the Employee Handbook (Policy and Procedure Manual).

Ø Use Universal Body Fluid Precautions and follow OSHA guidelines at all times.

Ø Communicate patient needs to the interdisciplinary team and coordinates care among the team.'
