Position – Registered Nurse, Patient Care Coordinator

Job Duration – 13 weeks with possible extension
Shift – Days – (06:00 AM to 02:00 PM)
40 hrs./week including every other weekend
Location – Hillsboro, OR 97124
Requirements for Patient Care Coordinator:
- Active RN License (OR State)
- Recent 1+ yr(s) of Care Coordination and Care Management experiecne as an RN
- BLS Certification through AHA
- Fully Covid Vaccinated & Boosted, if eligible.
