We are a recruiting company EasyHiring looking for candidates for the position of RN Care Manager for our partners
Company is adding a RN Care Coordinator to their team located in state MN area. The RN Care Coordinator would be working with members utilizing medical assistance under the age of 65 that have been determined disabled by the State or Federal Government.
This position will support members by assisting with appointments, resources, crisis situations/problem solving and assisting with complex care needs.
*Position will require visits both in home and in the community.
Schedule is full time; Monday - Friday.
Starting rate is up to $70 per hour depending on experience.
