Job Title: RN - Home Health

Job Location: Hayward, CA 94545

Job Type: Contract 13 weeks

Job Duties:

• Assess patient and family needs for home care and develop care plans.
• Administer treatments and medications as prescribed by physicians.
• Communicate with physicians and community health professionals to coordinate care.
• Provide patient counselling and health care instructions.
• Monitor and evaluate patient progress and revise care plans accordingly.
• Act as Case Manager for assigned caseload to coordinate patient care.
• Instruct and supervise home health aide care.

Job Requirements:

Experience

• 1 year of Home Heath experience
• Graduate of an accredited school of nursing with 1 year of recent acute care experience
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Good typing skills
• Wound care, wound vac, catheter, acute care experience
• Doesn't need CA license at time of submittal but will have to be willing and able to obtain.

Education

• Graduate of an accredited school of nursing

License, Certification, Registration

• Current nursing license and CPR certification
• Covid Vaccination.

Schedule:

• 40 hours scheduled
• Tues-Sat ideally or Mon-Fri with possible weekends on request

Pay: $90.00 per hour

• 5x8
