Job description

A. Completion of the curriculum requirements for an Associate’s degree in nursing, possession of a valid, current, Oklahoma License as approved by the Oklahoma Board of Nurse Registrations and Nursing Education to practice professional nursing. Must possess a valid, current, Oklahoma License as an RN by the Oklahoma Board of Nurse Registration.

B. Demonstrated knowledge of the integrated care philosophy.

C. Demonstrated skill in working with people, in treating social and emotional problems, in following oral and written instructions, and in exercising good judgment.

D. Must maintain a valid driver’s license and personal automobile insurance.

II. RESPONSIBILITIES:

A. Processes referrals to the Health Home, conducts initial appointment, and gathers all pertinent health and mental health information.

B. Provides professional nursing services to Health Home members and coordinates care with external providers.

C. Participates in the comprehensive treatment planning in partnership with behavioral health case manager.

D. Supervises Wellness coaches and meets with members regarding wellness goals.
