Sunnyside Care Center is actively seeking Registered Nurse (RN) , this is a full time position. Come in and see our new renovations and meet our Team! Excellent salary! SIGN ON BONUS ($5000) We currently have positions available.
Sunnyside Care Center is Onondaga County’s leader in sub-acute and short term rehabilitation. Sunnyside Care Center is an 80 Bed Short Term Sub-Acute Rehabilitation and Long Term Care Facility located in East Syracuse, NY. Currently seeking full,part time and weekend per diem positions

REQUIREMENTS:
. Current New York State Registered Professional Nurse license that is in good standing.
. Leadership qualities.
. Preferred Candidate will have 1 year of LTC (long term care) & skilled nursing and rehab experience in a supervisory capacity.
. Ability to address resident and family concerns.
. Excellent communication skills
. Ability to assess the health status of the Long-Term setting resident.
. Available to work full-time shifts, or as needed in emergency situations.
. Dependability/Reliability.

REPORTS TO:
Director of Nursing

RESPONSIBILITIES INCLUDE, BUT ARE NOT LIMITED TO:
1. Supervision of resident care by making rounds of the facility.
2. Document RN assessments on residents with change in condition and when assessment is warranted.
3. Complete staff evaluations for shift personnel after submitting to the Nurse Manager /
Director of Nursing for review.
4. Assist with nursing orientation for new employees and assist with educational in-services
as requested by the Assistant Director of Nursing /Director of Nursing.
5. Perform Quality Assurance activities and Audits as requested by the Director of Nursing.
6. Communicates pertinent resident information to the attending physician and Nurse
Managers.
7. Prepares and signs 24 hour shift reports.
8. Investigates and reports complaints regarding nursing services. Initiates written
investigation reports pertaining to any incidents and obtains staff statements of such
incidents within prescribed timelines and communicates findings to the Director of Nursing and the Administrator as appropriate.
9. Assures that resident rights are protected, that individual resident preferences are
identified and that all residents are treated in a caring, dignified, and respectful manner.
10. Becomes familiar with resident Care Plans and assure they are followed, makes and
communicates adjustments as needed.
11. Relays to staff any new Policies and Procedures, and assures that they are adhered to.
12. Shift specific duties as assigned by Director of Nursing.
