Job Summary:
The Referral Coordinator I provides direct support to patients and medical staff by managing referrals of clinical care to services outside of Tanana Chiefs Conference (TCC) Health Division. The Referral Coordinator I serves as a vital communications link between patients, their primary care providers, and outside providers of referred care to assist with timely appointment scheduling, transmission of medical records, coordination of travel, verification of Purchased Referred Care (PRC) payment arrangements, receipt of referral results, and coordination of follow up care and communication. This position works in a multi-disciplinary team-based system of care providing care for specific regions and villages served by TCC. This affords the job incumbent with the opportunity to engage with patients from across the TCC region and have a positive impact on the patient experience and health outcomes.

Representative Duties: Under the direct supervision of a Lead RN the job incumbent will be assigned from the following representative duties:

1. Actively manages referrals of care on behalf of primary care patients to facilitate a smooth transition from the medical services provided by Tanana Chiefs Conference to an outside provider and back to TCC. Receives referral orders from primary care providers (PCPs) at TCC and initiates communication between patients and referral providers both locally and outside of Fairbanks. Assists patients with appointment scheduling, medical records transmission, travel coordination, verification of PRC payment arrangements, and follow up on referral results. Closes the loop on referrals to ensure that both the patient and their PCP receive results in a timely fashion that allows the highest quality care and achievement of the desired health outcomes.

2. Works directly with PCPs to initiate coordinate referrals for both routine screening services, chronic care, and acute medical needs. Identifies, tracks, and follows up with patients who need support in order to engage in care that is needed to restore and maintain optimal health and wellness.

3. Frequent communication by phone, electronically, and in person with patients, medical providers, nursing staff, and other support staff. Manages the receipt and transmission of protected health information (PHI) as needed to facilitate care.

4. Meets with patients in person or via videoconference either to communication important referral information and answer questions, or the host telemedicine visits with remote providers of referred care.

5. Performs other job-related duties as assigned.


Minimum Qualifications


Minimum Qualifications:

Incumbents may meet minimum qualifications for this position with the following combinations of education, training, and experience

  • High School diploma or GED required. Post-secondary degree in a health care related field preferred.
  • Proficiency in office procedures, record keeping, written and verbal communication, and task & time management gained through education and training or through prior work experience.

Knowledge, Skills and Abilities:

1. Familiarity with medical terminology and medical procedures.1. Proficiency with PC based office productivity software (Microsoft Office, Outlook, etc.)

2. Ability to operate standard office equipment.

3. Ability to maintain patient confidentiality, interact effectively with patients and fellow staff and communicate clearly.

4. Strong critical thinking and decision making ability.

5. Excellent verbal and written communication skills.

6. Must be dependable, prompt, motivated, and able to work under minimum supervision.

7. Strong affinity for team-based work and collaborative problem solving.

Supervision: This position does not have any supervisory responsibilities.

Physical demands:
Position is mostly sedentary with extended periods sitting at desk/computer. Job incumbent must be able to lift and carry up to 50 pounds. Incumbent must be able to work efficiently in a busy and sometimes stressful environment. Shared office space in a clinical setting.

Work environment: Work is performed in a clinic responsible for treating patients with a wide variety of medical problems. The employee may be exposed to communicable disease during performance of duties.
