Agate's Housing Programs Department serves households who are experiencing or at risk of homelessness in Hennepin County. Our programs include Prevention, Diversion, Coordinated Entry Assessments, Fast Track, Rapid Rehousing, and Long-term Supportive Housing for households with children and adults without children. By offering programs of various intensity and length, we hope to be able to provide the level of service each household needs to be successful. The goal in all of our housing programs is to end homelessness by assisting households in stabilizing their housing and achieving self-sufficiency.

The Rapid ReHousing Lead Case Manager is a key member of the Rapid ReHousing Team who supports onboarding of new staff, mentorship of current staff, and partners with the Program Manager to contribute to program enhancements. The Team Lead is also responsible for providing ongoing, structured case management services to households participating in the Rapid ReHousing program.

PRIMARY DUTIES AND EXPECTATIONS

  • Provide consultation, assistance, and mentorship to Team Members related to participant scenarios, day-to-day responsibilities, and core components of housing case management.
  • Support consistent communication with Team Members related to program policies and practices.
  • Partner with the Program Manager to shape and implement program enhancements.
  • Support the Program Manager and Team Members with program operations that align with the Team Lead's unique strengths and evolving programmatic needs.
  • Work with the Program Manager to prepare for program reports and site visits.
  • Assisting with intern supervision.
  • Assisting with onboarding and training of new staff.
  • Collaborate with other staff members across the agency, including other Team Leads, to elevate Team Member voices and contribute to agency initiatives.
  • Model proficient skills for required documentation, including timely case noting and data collection in Client Track.
  • Maintain a caseload and provide housing case management to assist households with maintaining housing, developing goal plans, and accessing community resources.
  • Work with families and single adults experiencing homelessness in Hennepin County to assist them with transitioning from homelessness to stable housing.
  • Work respectfully with households with diverse identities and experiences including households who may experience historical and generational trauma; past or current domestic violence; challenges with mental, physical, developmental, and chemical health; and persistent low income.
  • Provide landlord-tenant support, mediation, and tenant education and offer consultation for Team Members to support housing stability for participants.
  • Manage multiple sources of funds to assist participants and maintain accurate records of expenditures.

Performs all other duties, as assigned.

Position consists of approximately 50% direct client contact*, and 50% indirect client-related work (i.e. documentation, communicating with collateral partners, preparing for client meetings, attending trainings and community events).

  • Please note that currently due to COVID-19, safe social distancing measures are in place for case management services.

PHYSICAL REQUIREMENTS FOR THIS POSITION

Sedentary work - Sitting most of the time with occasional walking. Exerting up to 10 pounds of force occasionally.

Must be able to attend home visits and apartment showings with clients, including units that may only be accessible by climbing stairs.

REQUIRED QUALIFICATIONS

  • Must have valid Driver's License and pass a DHS NetStudy 2.0 background check and a driving record background check.

Experience that gives applicants the required skills, abilities, and perspectives listed above are commonly gained through one or more of the following:

  • Personal experience with homelessness;
  • Experience working, interning, or volunteering with people experiencing homelessness;
  • An Associates, Bachelor's, or Master's degree in a relevant field such as Social Work (with accompanying licensure), Human Services, or Family Studies.

This position requires use of your own personal vehicle for Agency business and will be required to maintain liability insurance coverage of at least $100,000 per person and $300,000 per occurrence. In addition to criminal background studies, applicants must have a valid Driver's License and pass a driving record background check.

PREFERRED QUALIFICATIONS

  • Two years' experience working with people experiencing homelessness.
  • Fluency in languages used by our clients (Spanish, Amharic, Oromo, Somali, ASL)

AGENCY COVID-19 REQUIREMENTS

Agate Housing and Services maintains a COVID-19 preparedness plan that mitigates the transmission and spread of COVID-19. Programs also maintain practices to ensure the safety of staff, clients, residents, and community members.
