Position: Quality Improvement Specialist
Department: Clinical Services
Reports To: Director of Quality Improvement
FLSA Status: Exempt

Position Summary:
The Quality Improvement Specialist is responsible for projects at the ground level, ensuring that all initiatives are carried out in a manner that aligns with providing effective, high-quality, and patient -centered care.

Duties and Responsibilities:
Quality Improvement
  • Support the Director of Quality Improvement by facilitating and documenting subject matter expert knowledge during Quality Improvement program (QIP) Committee and PCMH Workgroups.
  • Facilitate, monitor, and document quality improvement projects that teams (individual care teams and interdepartmental teams) are testing and refining, primarily driven by QIP Workgroup projects.
  • Monitor annual Quality Plan, monitoring progress and working with department heads to reach annual goals.
  • Work across departments to ensure integration of and generate buy-in for quality initiatives.
  • Coach different members of the care team to improve their clinical quality metrics and increase the number of providers and departments meeting quality initiatives and all quality goals.
  • Collaborate with managers and departments as needed on quality improvement initiatives.
  • Responsible for population health outreach initiatives through CareMesage platform to promote preventative care and close gaps in care.
  • Other duties and responsibilities as assigned.

Data management and Interpretation
  • Build, run, and monitor quality management reports in the electronic medical record. These reports provide data utilized for grant reporting, population health outreach, and coaching.
  • Support Director of Quality with data requests and communications, such as weekly quality contest updates
  • Administer patient satisfaction results and dissemination of plan.

Pay for Performance
  • Serve as quality representative at MCO meetings and provide updates to Director of Quality Improvement.
  • Oversee medical records requests (“chart chases”) for HEDIS and ensure that partners receive information in a timely manner.
Experience:
  • Master’s Degree in a healthcare discipline with at least two years of experience in quality improvement in a healthcare setting required.
  • Passion for data and measurement tasks, as well as translating and disseminating data to all staff with varying backgrounds.
  • Bilingual in English/Spanish or English/Polish preferred.
  • Previous experience with Electronic Health Records preferred.
  • Strong ability to us MS Office programs, including MS word, Excel and Outlook.

Physical Demands:
  • Must be able to remain in a stationary position 50% of the time.
  • Must be able to move around the clinic site 50% of the time.
  • Constantly operates a computer, computer printer, copy machine, and telephone.
  • Occasionally positions self to maintain exertion of physical strength to move objects of 10 pounds from one level to another.
  • Must be able to transport from one site to another.
  • Must be able to cover other shifts as necessary.
