The Center for Alternative Sentencing & Employment Services (CASES) provides programs for individuals involved in or at high risk for involvement in the criminal legal system and operates one of only two outpatient mental health clinics (Nathaniel Clinic) in New York City specializing in the psychiatric care of this population. The Nathaniel Clinic also operates as a critical community provider in Central Harlem, where the rates of resident incarceration and psychiatric hospitalization are 300% and 47% higher than respective rates citywide. A core mission of the clinic and CASES is to address racial, economic, and health inequities in order to optimize every individual’s chance for their own success.

The Nathaniel Clinic offers a range of mental health care services, including individual and group psychotherapy, psychopharmacology, an in-house care management team, and a collaborative primary care relationship with the Institute for Family Health. These services will be expanded to include family, peer, crisis intervention and more robust substance use services as a result of CASES’ receipt in May of 2020 of a substantial grant from the US Substance Abuse and Mental Health Services Administration (SAMHSA) to operate the city’s only forensic Certified Community Behavioral Health Clinic (CCBHC). Many of the patients are referred to the clinic as part of an alternative to detention (supervised release) or alternative to incarceration program, or upon institutional release from a jail, prison or hospital.

All applicants must comply with CASES/NYC's vaccination policy. Individuals selected for employment cannot begin working until they provide verification that they are fully vaccinated against COVID-19 (with an FDA-authorized vaccine) or submit and gain approval for a Medical or Religious Exemption. Individuals approved for exemption must submit weekly PCR test results.

We believe that in-person operations are the best way to serve our clients, both directly and through essential operational support. This job requires work in our offices and/or the community.


Job Description
  • Provide evidence-based, trauma-informed, patient-centric and team-based psychiatric care for individuals with a wide range of clinical and psychosocial needs. Emphasis is on working collaboratively with the clinic’s social worker therapists to provide diagnostic assessment, psychopharmacology services, and complex care management
  • Mix of telepsychiatry (utilizing Zoom for Healthcare) and in-person services.
  • Become a NYC certified naloxone dispenser and educate patients/distribute naloxone kits as appropriate
  • Prescribe buprenorphine/naloxone for eligible patients (anticipated to start in clinic 2021)
  • In collaboration with the clinic nursing staff, identify and monitor primary care and medical specialty needs referred to appropriate providers
  • For patients who are receiving treatment services only from a psychiatrist (e.g., medication only, combined medication/therapy, outside therapist), completion of semi-annual treatment plans per NYS Office of Mental Health guidelines
  • Timely, clear and appropriate documentation of activities in the clinic’s EHR
  • Supervise psychiatric residents completing clinical rotations at the clinic with emphasis on issues related to working with individuals involved in the criminal legal system
  • Actively participate in weekly staff meetings, high risk meetings, treatment team meetings, and psychiatric provider meetings
  • Identify and participate in QI activities as interested and appropriate

Qualifications
  • M.D. or D.O. degree with active New York State license in good standing
  • Graduate of ACGME-accredited psychiatry training program
  • ABPN board-certified or board-eligible
  • Excellent diagnostic, clinical formulation and treatment skills
  • Flexibility to work successfully with a wide range of multi-disciplinary staff
  • MAT-waivered or ability to receive waiver within 3 months of hire
  • Experience working with individuals involved in the criminal legal system preferred
